Definitely think of answers to obvious questions like this prior to arriving at the interview. In your answer, explain what was going on, how it went wrong, and how you resolved the failure. Try and use real life, work experiences that involve personnel, finances, or outside organizations. Don't talk about a homework assignment/test/term paper - no one cares.
