Management is horribly disorganized. Processes are poorly documented or not documented at all, and almost never followed, which causes endless problems in an FDA regulated industry. This leads to a perpetual panic mode where engineers are forced to rush out fixes instead of write quality software. There are interesting problems to solve here but you'll be too busy putting out yet another fire to ever work on them. Stinginess with hiring budget has only made this worse; subject matter experts are often forced into mundane work beneath their ability because management won't hire a proper DevOps team to automate things that should be.

Lots of leadership changes, low morale due to lack of bonuses and some layoffs, this will get better over time.
