Out of date leadership/management approaches. Using heavy-agile (SAFE) has become quarter waterfall. Teams work in islands with cross team cooperation is indirectly discouraged by way of scrum ticket accountability -- destroys synergy. One result of this that a supportive teams such as DevOps has become defensive when ask for help or when problems are pointed.
