Question 1
There was general questions about me and previous experiences
The interview process was straightforward.
Phone Screen with HR
Phone Screen with Hiring Manager
Onsite Interview - 4 rounds
I got a verbal offer within 1 week. Followed by an email the next day. After 3 weeks of follow-up, the HR called me and said that they did not get approval to issue the offer letter from the finance team and they are putting a freeze on the position with me still tagged to it as the person hired. I never heard back from them and all my follow-ups went unanswered.
The process also wasted hours submitting background check documentation and immigration documentation for H1B and PERM.

Question 1
Describe a recent breach - what was the exact cause? How could it have been avoided?
The process took 4 months. I interviewed at Adobe (San Francisco, CA) in Oct 2018
Interview
Long and slow process. A recruiter reached out, then had a call with the hiring manager and a teammate. Then came onsite for an in-person panel interview and came onsite again for lunch with some of the day-to-day peers. Then radio silence for over a month, and finally follow-up news that they wanted one more call with a VP. So I dial into the conference call line and the VP no shows. Takes Adobe 2 weeks to reschedule and again the VP no shows. Six more weeks of back and forth with recruiting to figure out what's going on and then the role gets canceled. By far the worst interview experience of my life. The candidate experience was telling and I'm glad it happened as I avoided a disaster in going to Adobe and am now at a FAANG company in the bay.
