Far too few external hires, insular company culture, many underqualified individuals in senior-level roles due to long tenure, many scandals in recent years have taken a toll on employee morale and led to a culture of 'playing the victim,' bureaucratic structure of company stifles innovation, management slow to recognize/respond to information that contradicts current business strategies, current emphasis on efficiency improvements and cost cutting has drastically restricted opportunities for job promotion and eliminated nearly all employee job percs like optional training and team offsite meetings, compensation is below market

Ineffective and ignorant leadership, single person running the show from US (for knowledge services) and he is equally clueless, no work, only politics, below par senior management
